Hazard or incident reports received by MPDC are entered into the Records Management or Customer Request Management systems and assigned to the managers of the service or area the report is related to. If you chose to lodge your report anonymously then our usual process of contacting you to discuss a resolution or let you know how we have dealt with your issue will not happen. However what you identified in your report, without your personal details, and how we resolved it may be published on our website or internally to help improve our services.
