Safety reports received by MPDC are entered into the Safety Management System. If you chose to lodge your complaint anonymously then our usual process of contacting you to discuss a resolution of let you know how we have dealt with your issue will not happen. However what you identified in your complaint, without your personal details, and how we resolved it may be published on our website or internally to help improve our services.
Use this form to notify the MPDC health and safety team of a hazard or an incident where someone was injured or could have been injured. If a serious injury has occurred or you think there is an imminent threat to safety, please call emergency services (111) as appropriate and also advise MPDC on 0800 746 467.
